Job Summary

The Methodist Hospital is seeking a skilled RN Case Manager to join our team. As a Case Manager, you will work with patients and families to develop a plan that optimizes functional and self-care abilities, facilitates system access and appropriate utilization of services, and promotes quality and cost-effective outcomes.

Responsibilities
  • Assess discharge needs of high-risk patients and identify resources to address those needs.
  • Coordinate referrals and document plans to ensure seamless care transitions.
  • Utilize community resources to support patients' post-hospital needs and track resource statistics to identify improvement opportunities.
  • Attend medical staff and other hospital committee meetings to report findings and stay current on requirements from JCAHO, HCFA, and other state/federal agencies.
  • Facilitate and provide education to hospital staff, physicians, patients, and caregivers to promote efficient resource utilization.
  • Perform admission and continued stay reviews, provide clinical information to payers, and discuss alternate levels of care with physicians.
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited program.
  • A minimum of 3 years of clinical experience in the area of case management.
  • A valid RN license to practice in the state of Texas or an active Multi-State RN Compact License.
  • Preferred qualifications include a Bachelor of Science in Nursing and experience in utilization review, resource management, discharge planning, or case management.
