We're seeking a highly skilled Primary Care Physician to join our team at ChenMed. As a key member of our clinical operations team, you will provide direct patient care and assessments in an ambulatory healthcare setting.
Responsibilities- Conduct geriatric assessments, medical histories, and physical exams to diagnose and treat patients.
- Develop and implement patient management plans, including referrals to specialists when necessary.
- Collaborate with multidisciplinary team members to analyze and evaluate healthcare delivery systems.
- Work with patients and families to transition to lower levels of care as soon as medically appropriate.
- Enhance patient and family relationships to maximize informed decision-making.
- Board certification in Internal Medicine or Family Medicine is preferred, with the ability to achieve certification within 2 years.
- Minimum of one year of clinical experience in geriatric, adult, or family practice settings.
