Job Summary:

Pacer Staffing is seeking a highly skilled Registered Nurse to fill the role of Care Manager RN. As a Care Manager RN, you will be responsible for providing exceptional patient care and ensuring the highest level of satisfaction for our clients.

Key Responsibilities:

  • Outreach and Enrollment: Conduct outreach to members to enroll them in the Care Management program, confirming eligibility for CM services.
  • Care Planning: Build working care plans, conduct medication reviews, complete assessments, and address care gaps.
  • Documentation: Document each case, which can take around 2 hours per case to complete.

Performance Expectations:

  • Call Volume: Make 20 calls per day.
  • Enrollment Goals: Achieve 10 enrollments per week.

Requirements:

  • Time Management: Possess excellent time management skills to work in a fast-paced environment.
  • Technical Skills: Have strong technical and documentation skills, with recent telephonic CM remote work experience with Managed Care and/or Medicare.

Preferred Qualifications:

  • Technical Skills: Strong technical skills, with knowledge of TruCare documentation system.
  • Language Skills: Bilingual abilities (Spanish/Chinese/English) are strongly preferred.

Education and Licensure:

  • Education: Associates degree.
  • Licensure: Must have a valid CA RN license.

Software Skills:

  • Microsoft Office: Proficiency in Microsoft Office Suites.

Work Schedule:

  • Work Hours: 8 AM to 5 PM PST.
