Job Summary:The Ambulatory Care Nurse will be responsible for providing comprehensive care to patients in an outpatient setting. This includes assessing patient needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless patient care.
Key Responsibilities:- Assess patient needs and develop individualized care plans
- Provide direct patient care, including administering medications, performing procedures, and providing education to patients and families
- Collaborate with healthcare teams to ensure seamless patient care
- Participate in quality improvement initiatives to enhance patient outcomes
- Document patient information accurately and thoroughly
- Baccalaureate degree in Nursing (highly preferred) or Diploma or Associates Degree in Nursing with plans to earn a baccalaureate degree
- Current Registered Professional Nurse in New York
- Basic Life Support Certification through the American Heart Association
- Fluent written and spoken English language skills
- ACLS certification
- Membership in pertinent professional organizations
