As a skilled RN Case Manager, you'll play a vital role in our mission by providing comprehensive care to patients in the comfort of their own homes.

  • Lead Patient Care Teams: Collaborate with multidisciplinary teams, including physical therapists, occupational therapists, and home health aides, to develop and implement personalized care plans that address patients' unique needs and goals.
  • Conduct Home Visits: Perform thorough assessments, administer skilled nursing visits, and provide ongoing care and case management to patients in their homes.
  • Communicate Effectively: Maintain open communication with patients, families, and healthcare providers to ensure seamless care coordination and timely interventions.
  • Manage Patient Records: Accurately document patient information, progress updates, and care plan changes in our electronic health record system.
  • Ensure Quality Care: Monitor patient outcomes, identify areas for improvement, and implement evidence-based practices to enhance the quality of care delivered.
Requirements and Qualifications

To succeed in this role, you'll need:

  • Nursing Experience: A minimum of one year of nursing experience, preferably in a home health or acute care setting.
  • Strong Clinical Skills: Excellent assessment, critical thinking, and problem-solving skills to provide high-quality patient care.
  • Effective Communication: Strong verbal and written communication skills to collaborate with patients, families, and healthcare teams.
  • Organizational Abilities: Ability to prioritize tasks, manage multiple patients, and maintain accurate records.
