Position Details
This role involves a commitment of 32 hours per week, primarily during evening shifts, including coverage every fourth weekend with rotating schedules between day and evening shifts.
Our pharmacists are responsible for delivering pharmaceutical care services to hospital patients, which encompasses oversight, monitoring, preparation, dispensing, and accurate documentation of patient medication therapies. They also provide essential drug information and ensure the integrity of information and dispensing systems. The ideal candidate will collaborate with the healthcare team to uphold compliance with federal, state, and local regulations regarding pharmaceuticals.
Key Responsibilities
- Accurately interprets and transcribes medical orders into computerized patient medication profiles, ensuring completeness and accuracy.
- Compounds and dispenses medications, including sterile and chemotherapy products, with precision.
- Manages the issuance of controlled substances to patient care areas, maintaining compliance with legal record-keeping requirements.
- Monitors drug therapy for potential contraindications, interactions, and appropriateness of medications and dosages.
- Provides pharmacokinetics consultations and adjusts drug dosing according to established protocols.
- Reviews culture and susceptibility data to ensure appropriate antibiotic use and recommends modifications as necessary.
- Identifies and reports suspected adverse drug reactions promptly and accurately.
- Maintains formulary integrity by minimizing nonformulary procurements and promoting effective drug therapy selection.
- Offers clinical consultation to healthcare practitioners, suggesting cost-effective therapeutic alternatives when appropriate.
- Documents all clinical activities and interventions comprehensively and accurately.
- Engages in quality improvement initiatives and medication use reviews, collecting data and maintaining necessary documentation.
- Participates in the development and delivery of educational programs for pharmacy, medical, nursing, and other staff members.
Qualifications
- Must hold a valid pharmacist license in good standing.
- Bachelor's degree in Pharmacy or PharmD is required.
- Experience in inpatient hospital pharmacy is highly preferred.
- Excellent communication skills, both verbal and written, are essential for interaction with patients and healthcare professionals.
- Strong customer service orientation.
- Proficient in utilizing various pharmacy information systems.
