About the Role:
The Clinical Liaison Specialist will work as a member of an interdisciplinary team to ensure comprehensive and accurate documentation of patient care. The successful candidate will have excellent critical thinking skills, be able to prioritize tasks effectively, and maintain a high level of professionalism in all interactions with patients, families, and healthcare staff.
Responsibilities:
* Facilitate hospice and palliative care referrals from various sources
* Conduct clinical assessments to determine patient needs and develop care plans
* Coordinate patient care across different settings, including hospitals, clinics, and home care
* Collaborate with healthcare staff to ensure seamless transitions of care
* Maintain accurate and comprehensive documentation of patient care
* Provide education and support to patients and families regarding care options and goals
Requirements:
* Associate Degree in Nursing or higher
* Current State of New Mexico or Compact State Nursing License
* BLS certification required at time of hire
* ACLS and PALS certifications preferred
* 1 year of hospice experience required; acute care experience strongly preferred
* Strong communication and interpersonal skills
* Ability to prioritize tasks effectively and manage multiple projects simultaneously
