Job Description
  • Assist the Branch Director in coordinating home health services to ensure delivery according to acceptable standards of practice.
  • Review and approve patient information submitted by Licensed Professionals during Start of Care, Recertification, Resumption of Care, or Evaluation visits.
  • Provide oversight of patient care and assist with case conferences, order reviews, and documentation requirements to meet regulatory standards.
  • Contribute to the overall company success of providing A Better Way To Care by facilitating relationships between physicians, referral sources, patients, caregivers, and employees.
Qualifications
  • Graduate of an accredited school of professional nursing and licensed in the state of employment and where practicing.
  • A minimum of two years nursing experience and one-year experience in home health.
  • Good understanding of Federal, State, and local laws/regulatory guidelines governing the operation of a Home Health agency.
  • Skilled competency with technology - general office equipment and EMR platforms, preferably HCHB.
  • Valid state driver's license and automobile liability insurance.
  • Ability to drive an automobile in all types of weather conditions.
