Job Description

In this role, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that meet the unique needs of each patient. You will also participate in quality improvement initiatives, collaborate with interdisciplinary teams, and contribute to the development of policies and procedures.

Responsibilities
  • Provide direct patient care services, including assessments, treatments, and education.
  • Collaborate with interdisciplinary teams to coordinate patient care services and ensure seamless transitions between departments.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Act as a patient advocate, ensuring that patients receive high-quality, respectful care that meets their cultural and linguistic needs.
Requirements

To be successful in this role, you must have:

  • A Bachelor's degree in Nursing (BSN) from an accredited institution.
  • A minimum of 8 years of experience in a related field, preferably in ambulatory care or community health.
  • A valid Registered Nurse (RN) license in good standing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
Estimated Salary Range

$80,000 - $120,000 per year, depending on experience.
