Job Summary:

UnitedHealthcare is seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in improving the health and well-being of our members by providing personalized support and guidance.

Key Responsibilities:

  • Engage with members to assess their health needs and develop personalized care plans
  • Conduct comprehensive assessments to identify health concerns and develop strategies to address them
  • Collaborate with healthcare providers to ensure members receive the care they need
  • Provide education and support to members on healthy behaviors and disease management
  • Identify and connect members with community resources and services
  • Monitor member progress and adjust care plans as needed

Requirements:

  • High school diploma or equivalent required; 7+ years of case management experience or equivalent required
  • 3+ years of experience working in a field-based setting
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, Excel, and multiple web applications
  • Valid driver's license in the state of Ohio and current automobile insurance
  • Access to reliable transportation and the ability to travel locally, up to 100 miles round trip and up to 50% of the time to client and/or patient sites

Preferred Qualifications:

  • Community Health Worker (CHW) Accreditation
  • Experience working in managed care
  • Knowledge of Medicaid/Medicare population
  • Knowledge and/or experience with behavioral health or substance use disorders
