Physical Therapist

At Solution Based Therapeutics, we are seeking a dedicated and compassionate Physical Therapist to join our team at a skilled nursing facility.
We offer a competitive salary range of $85,000 - $110,000 per annum, depending on experience.
Key Responsibilities- Patient Evaluation and Assessment:
- Conduct comprehensive evaluations of patients' physical and functional abilities upon admission and throughout their care.
- Develop individualized treatment plans based on assessment findings, patient goals, and physician recommendations.
- Treatment Implementation:
- Provide evidence-based therapeutic interventions, including exercises, mobility training, and pain management techniques.
- Utilize specialized equipment and techniques to improve strength, balance, coordination, and overall mobility.
- Monitor patient progress and adjust treatment plans as needed.
- Collaboration and Communication:
- Work closely with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to ensure coordinated patient care.
- Communicate regularly with patients and their families regarding progress, goals, and discharge planning.
- Documentation and Compliance:
- Maintain accurate and timely documentation in accordance with facility policies, state, and federal regulations.
- Ensure compliance with Medicare, Medicaid, and other third-party payer requirements.
- Education and Advocacy:
- Educate patients and their families on exercises, safety precautions, and strategies to maintain progress post-discharge.
- Advocate for patients to achieve their highest level of independence.
- Facility Support:
- Participate in team meetings, quality improvement initiatives, and staff training.
- Assist in maintaining a safe and clean therapy environment.
