**About Us**

AmeriMed Infusion Pharmacy is a leading provider of home infusion therapy services, dedicated to helping patients safely transition care to their home and alleviate long hospital stays.

As a Care Transition Nurse with AmeriMed, you will play a vital role in our mission to provide total care for our patients. You will work directly with physicians to manage various diseases and conditions in a homecare setting, educating patients and caregivers on effective therapy management and providing ongoing oversight, coordination, and support throughout the therapy process.

**Responsibilities**

You will be responsible for:

  • Following patient referrals and coordinating quality home care services
  • Determining patient eligibility for infusion services
  • Communicating effectively with healthcare providers and facilities to generate referrals and admissions
  • Developing professional working relationships with healthcare teams to provide value-added counsel, teaching, and problem-solving
  • Analyzing eligibility for infusion services and providing general guidance on Medicare and Medicaid guidelines, insurance plan benefits, financing options, and organizational policies

**Requirements**

To be successful in this role, you must have:

  • Graduate of an accredited school of nursing
  • Experience as a Care Transition Nurse or similar role, sales, or marketing
  • Hospital and/or homecare nursing experience
  • Current RN or LPN license to practice in the state serviced
  • Ongoing CPR certification and ability to perform CPR
  • Strong assessment, critical thinking, judgment, creativity, and decision-making skills
  • Excellent communication skills, both verbal and written

**Salary Information**

The estimated salary for this position is $27.45-$39.80 per hour, based on national averages and location considerations.
