Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Language Pathologist to join our team in North Kansas City, Missouri.

Job Summary:

We are looking for a compassionate and experienced Speech Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that address the unique needs of each patient.

Responsibilities:
  • Develop and implement individualized plans of care for patients with speech and language disorders.
  • Provide home-based skilled speech therapy care to patients, including assessment, treatment, and education.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with agency policies and regulations.
  • Participate in ongoing education and training to stay current with best practices in speech therapy.
Requirements:
  • Current license to practice as a Speech Therapist in the state of employment.
  • Minimum one year of experience as a Speech Therapist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job, including traveling to patient homes.
