Home Health Physical Therapist

LHC Group is seeking a highly skilled Physical Therapist to join our team in Rocky Mount, North Carolina.
About the Role:The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. They will work closely with the physician to develop a treatment plan, perform interventions to improve patient well-being, and evaluate patient progress.
Responsibilities:- Evaluate patient functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
- Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of North Carolina.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
