We are seeking an experienced Social Worker to join our Health Services Life Enrichment Team at Givens Estates, a nationally recognized nonprofit life plan community. This full-time position offers a unique opportunity to make a meaningful impact on the lives of our residents.

About the Role:

  • Develop and implement personalized care plans for residents in our skilled nursing and rehab facility, as well as assisted living residents.
  • Conduct social assessments, provide referrals, and coordinate discharge planning to ensure seamless transitions.
  • Advocate for residents' rights, address psychosocial needs, and promote leisure activities.
  • Work collaboratively with healthcare professionals, families, and other stakeholders to deliver high-quality care.

Requirements and Qualifications:

  • Bachelor's degree in a human services field (e.g., social work, psychology, or counseling).
  • At least 2 years of experience working with older adults, preferably in a healthcare setting.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a team, with flexibility to accommodate varied schedules.
