Job Summary

The Clinical Care Leader (CCL) is a key member of our healthcare team, responsible for delivering high-quality nursing care to our residents. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.

Key Responsibilities
  • Utilize the nursing process to assess, diagnose, plan, implement, and evaluate individualized care plans for residents.
  • Collaborate with the inter-professional team to plan, implement, and evaluate care.
  • Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations.
  • Utilize technology to enhance quality resident care and collaborate with inter-professional team members.
  • Delegate to and supervise unlicensed personnel, assuming overall nursing accountability for a resident assignment.
  • Serve in leadership roles on departmental and organizational task forces and committees.
Requirements
  • Bachelor's degree in nursing preferred.
  • Minimum of two years long-term care or geriatric nursing experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring.
  • Obtains and maintains required department-specific competencies and certifications.
