Job Description

We are seeking a skilled Registered Nurse to join our team at BAYADA Home Health Care. As a Home Health Nurse, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Responsibilities:
  • Develop and implement personalized care plans in accordance with client needs and goals.
  • Conduct home visits to provide high-quality patient care and support.
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
  • Monitor client conditions and report changes to clinical or client services managers.
  • Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
  • Collaborate with healthcare teams to ensure seamless care coordination and management.
Requirements:
  • A current license as a Registered Nurse in New Hampshire.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills and solid computer skills, with prior experience with electronic medical records (EMR) preferred.
