Job SummaryWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, medical history reviews, and health education sessions with our patients. You will work collaboratively with our team to provide comprehensive care and support to our patients, and will have the opportunity to make a direct impact on the health and well-being of others.
Responsibilities- Conduct home risk assessments, medical history reviews, and health education sessions with patients
- Work collaboratively with physicians, case managers, and other healthcare professionals to provide comprehensive care and support to patients
- Develop and implement individualized care plans to address patient needs and goals
- Provide health education and counseling to patients and their families
- Collaborate with primary care physicians to ensure seamless care coordination
- Master's degree in Nursing required
- Current RN and NP licensure in state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent verbal and written communication skills
