The OpportunityWe're seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team in Sun City, AZ. As a key member of our outpatient practice, you'll play a critical role in delivering high-quality, comprehensive primary care to our geriatric patient population.
As a value-based care professional, you'll have the opportunity to work in a collaborative, team-based environment that's designed to support your success. You'll partner with our medical director and care team to develop and implement innovative care models that drive better health outcomes.
Responsibilities- Provide best-in-class comprehensive primary care in an outpatient setting
- Partner with the care team to leverage our coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change
- Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population
- Work in a state-of-the-art practice setting designed for a team-based practice model
- Enjoy a leadership team that values your input and understands the importance of work-life balance
- At least 2 years of primary care experience required
- Currently licensed or ability to obtain licensure in the state of Arizona
- The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups
- Can thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes
