LHC Group is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to patients in need.

About the Role:
  • Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
  • Develop and implement treatment plans to improve and enhance patient well-being.
  • Collaborate with healthcare teams to ensure comprehensive care and support for patients.
Responsibilities:
  • Evaluate patient functional status and physical therapy needs.
  • Assist physicians in developing therapy plans of care.
  • Observe and report patient responses to treatment and any changes in condition.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
  • Instruct and inform healthcare teams, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.
Requirements:
  • Current Physical Therapy licensure in state of practice.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to dependable transportation.
