Delivers nutritional care for patients in both in-center and home settings, aiming to enhance the nutritional health of patients and optimize clinical results.

Acts as an integral member of the multidisciplinary healthcare team, upholding the commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) initiatives.

Engages actively in process enhancement activities that support patients in reaching their personalized health objectives as outlined by their healthcare provider.


KEY RESPONSIBILITIES:


Evaluates patients' understanding of dietary needs related to kidney health and provides tailored education that aligns with their learning preferences, emphasizing the connection between nutrition and kidney function.


Offers detailed, patient-specific education to patients, families, and caregivers regarding nutritional status, adequacy measures, fluid management, and all nutrition-related laboratory results pertinent to their health conditions.

Conducts comprehensive nutritional assessments and collaborates in interdisciplinary evaluations.


Works with the healthcare team to create a personalized, comprehensive care plan that addresses the patient's needs, adhering to established federal and applicable state guidelines.

Calculates dietary prescriptions following standard practices.

Documents progress notes for all chronic dialysis patients in accordance with established guidelines.

Ensures complete and accurate documentation of all relevant information in medical records, complying with established policies and regulations.


Provides ongoing nutritional assessments and outcome-focused counseling to help patients achieve and maintain optimal nutritional health.


Identifies patients at risk of malnutrition and collaborates with the healthcare team to determine suitable interventions and resources.

Reviews each patient's albumin levels and body weight monthly. Implements care plans with the involvement of patients, families, and caregivers.


Monitors adherence to nutrition therapy, adjusting plans as necessary to maintain effective nutritional health, documenting any barriers to achieving goals and implementing strategies to address these challenges.

Collaborates with the healthcare team on Urea Kinetic Modeling recommendations to fulfill the physician's prescription of Kt/V.


Partners with the interdisciplinary team on managing and advising regarding mineral metabolism, renal bone disease, anemia, nutritional status, psychosocial aspects, vascular access, treatment modalities, rehabilitation, and sodium/volume control.

Follows established algorithms and medical record policies.

Reports on nutrition QAI outcomes and engages in the interdisciplinary QAI program.

Collaborates with team members to provide Pre-ESRD education as appropriate.


When necessary, coordinates with Case Managers to develop educational plans for patients and identifies candidates for nutritional supplement referrals as per disease management agreements.


REVIEWS NUTRITION-RELATED LAB RESULTS:
Advises patients and/or caregivers and formulates appropriate actions.

Suggests treatment modifications to the interdisciplinary team as needed.

Communicates lab results and recommended treatment changes to healthcare providers in various settings (e.g., hospitals, short-term and long-term care facilities).Participates in relevant staff meetings (e.g., general staff, quality improvement), care plan meetings, and others as applicable.

Consults with the Corporate Dietitian regarding nutrition-related concerns as necessary.

Maintains and enhances knowledge and skills for a competent and innovative practice.

Ensures dietetic registration and continuing education hours are maintained as required by the American Dietetic Association and state regulations where applicable.

Collaborates with the interdisciplinary team to ensure all work areas are safe and organized.

Other nutrition management responsibilities as assigned.

Performs additional related duties as required.

EDUCATION AND REQUIRED CREDENTIALS:
Registered Dietitian as per Commission on Dietetic Registration

Board Certified Specialist in Renal Nutrition is encouraged.

Current state licensure if applicable.

EXPERIENCE AND SKILLS:
A minimum of 1 year of professional experience in clinical nutrition as a Registered Dietitian.

Previous experience in renal nutrition is preferred.


Demonstrated leadership competencies and skills for the position, including the ability to educate, strong organizational skills, excellent communication, customer service, relationship development, results orientation, team collaboration, and decision-making.

Able to perform responsibilities with minimal supervision.

Basic computer skills are required.
