Job Summary:
Signature Healthcare is seeking a skilled Physical Therapist to join our team at Brockton Hospital. As a Physical Therapist, you will play a vital role in the rehabilitation of patients in an outpatient setting. Your primary responsibility will be to evaluate and provide direct patient care per physician orders, with the goal of rehabilitating patients to achieve stated goals. This is a challenging yet rewarding role that requires a high level of expertise and compassion.
Key Responsibilities:
Conduct thorough initial evaluations on each patient referred, appropriate to the patient's diagnosis according to established procedure and physician order.
Provide timely documentation at the time of evaluation and maintain accurate records.
Develop and implement effective treatment plans, including clear daily and progress reporting.
Instruct and assess patients in a variety of functional activities, utilizing techniques specific to the age group serviced.
Perform and assist patients through a variety of therapeutic modalities and techniques per established procedures.
Reassess and recommend modification of the treatment plan and goals as needed.
Meet with the members of the patient's care team in development and implementation of assessments, care plans, and discharge planning.
Instruct patient care staff on therapeutic techniques and equipment to facilitate the carryover of the therapy plans in daily activities.
Delegates treatments to PTA's to improve flow and productivity.
Provides ongoing patient, family, and caregiver education.
Presents with a positive attitude and demeanor at all times, adhering to the WE CARE standards.
Proficient in the use of computerized documentation and utilizes appropriate outcome measures.
Participates in departmental Lean activities and departmental projects/initiatives/year.
Provides clinical supervision to affiliated students under guidelines established by the participating educational institutions.
May participate in the mentoring program.
Establishes daily schedule, treatment plans, and priorities to service assigned patients.
Maintains established policies and procedures, quality assurance, safety, and infection control.
Commits to recognizing and respecting cultural diversity for all customers (internal and external).
Communicates effectively with internal and external customers with respect to differences in cultures, values, beliefs, and ages, utilizing interpreters when needed.
Requirements:
Generally works independently within department policies, however, will consult supervisor/manager regarding unusual problems or cases.
Follows established policies and procedures, quality assurance, safety, and infection control.
Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
Education/Experience/Licensure/Technical/Other:
Education: Bachelor of Science in Physical Therapy or Master of Science in Physical Therapy/DPT.
Experience: 5 years of clinical experience or previous experience is not required outside clinical requirements.
Certification/Licensure: Registration/Licensing in the State of Massachusetts is required.
Software/Hardware: Basic proficiency in computerized documentation and software used in the department.
Other: Basic CPR training will be provided.
