About the Role:

We are seeking an experienced Senior Registered Nurse to join our Ambulatory Care team at County of Ventura. The ideal candidate will possess a strong understanding of Enhanced Care Management services and be proficient in managing patient care with minimal supervision.

Key Responsibilities:

  • Assess and identify nursing care needs for patients
  • Develop and implement patient education programs
  • Establish effective working relationships with physicians, patients, and co-workers
  • Maintain accurate records and documentation
  • Participate in quality assurance processes and committee activities

Requirements:

  • Three years of full-time professional registered nursing experience, including two years in an assigned specialty area
  • Current, valid California RN license and BLS certification
  • Ability to work effectively in emergency situations and maintain confidentiality of patient information
