Job Summary

We are seeking a skilled and compassionate Home Health Nurse to join our team at LHC Group. As a Licensed Practical Nurse in Home Health, you will play a vital role in providing high-quality patient care and performing technical skilled care in the patient's home.

Responsibilities
  • Provide quality patient care and perform technical skilled care in the patient's home, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Instruct the patient's family in the care of the patient and maintenance of a healthy environment for the patient.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
