Job Summary

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Communicate effectively with patients, families, and healthcare team members
  • Participate in ongoing education and training to stay current with best practices in hospice care
Requirements
  • Graduate of an accredited LPN program
  • 1 year of experience in hospice care
  • American Heart Association BLS certification
  • NY LPN licensure
