RN Clinical Manager

We are seeking a highly skilled RN Clinical Manager to join our team at CenterWell Home Health. As a key member of our clinical leadership team, you will be responsible for overseeing the delivery of high-quality patient care services in a home health setting.
Key Responsibilities- Coordinate and oversee all direct care patient services provided by clinical personnel, ensuring compliance with regulatory requirements and Company policies.
- Develop, implement, and analyze clinical operations initiatives to improve patient outcomes and quality of care.
- Conduct assessments and reassessments of patients, updating care plans and interpreting patient needs to ensure effective care delivery.
- Manage the assignment of caregivers and ensure timely and effective delivery of care services.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
- Monitor and analyze clinical documentation to ensure compliance with regulatory agencies and requirements of third-party payers.
- Participate in performance improvement activities and maintain ongoing clinical knowledge through internal and external training programs.
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years of experience as a Registered Nurse, with at least one year of management experience in a home care, hospice, or equivalent environment.
- Home health experience is required.
- Management and people leadership experience is required.
- OASIS experience is required; OASIS certification (HCS-O, COQS, and/or COS-C) is preferred.
- Homecare Homebase (HCHB) experience is required.
- CMS PDGM billing knowledge and/or experience is preferred.
