Job Title: Travel Nurse RN

Cynet Health is seeking a skilled Travel Nurse RN to join our team. As a Travel Nurse RN, you will provide high-quality patient care in a home health setting.

Job Summary:

The Travel Nurse RN will administer skilled nursing care to patients on an intermittent basis in their place of residence. This will be performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.

Responsibilities:
  • Conduct thorough nursing assessments on admission and re-evaluate patient's nursing needs on succeeding visits.
  • Initiate and update the plan of care for SOC (Start of Care), ROC (Resumption of Care), RECERT (Recertification of Care), and DC (Discharge) using Medicare OASIS Forms.
  • Provide services or treatments requiring substantial and specialized nursing skill according with the written patient care plan and the signed plan of treatment.
  • Case manage all therapies: LPN, PT, OT, ST, and CNA.
  • Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Process orders and notify physician of patient needs and changes in condition.
  • Conduct patient care conferences on patient assigned to his/her care.
  • Be available for on-call duty nights, weekends, and holidays, as assigned.
Requirements:
  • Graduate of an accredited school of nursing.
  • License in good standing.
  • One year of recent experience in Home Health.
  • AHA BLS required.
