Job Summary

Tidelands Health is seeking a skilled Registered Nurse to join our team as a Nurse Care Manager. In this role, you will be responsible for managing individualized patient care, promoting and restoring patients' health through the nursing process, and collaborating with physicians and multidisciplinary team members.

Key Responsibilities
  • Conducts Patient Assessments: Conducts an individualized patient assessment, prioritizing data collection based on the patient's immediate condition or needs within the specified timeframe, in collaboration with other healthcare team members.
  • Provides Patient Care: Performs appropriate treatments and administrations of medication, immediate and ongoing, as ordered by the provider, in an accurate and timely manner.
  • Delivers Therapeutic Interventions: Performs therapeutic nursing interventions and delivers care with appropriate age and cultural competence, as established by individualized needs and plans of care for the patient and their family.
  • Provides Patient Education: Provides individualized patient and family education, customized to the patient's and their family's needs.
  • Documents Patient Progress: Documents patient condition, assessment findings, and physical and psychosocial responses to nursing interventions and progress towards problem resolution.
  • Initiates Emergency Measures: Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
  • Maintains Confidentiality: Maintains confidentiality and provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient's and family's diversity, preserving their autonomy, dignity, and rights.
  • Communicates Patient Symptoms: Observes and interprets patient symptoms and communicates them to providers and other staff during shift change.
Requirements
  • Education: Associates Degree in Nursing (ADN) from an education institution accredited by the National League of Nursing Accrediting Commission (NLNAC) or Commission on Collegiate Nursing Education (CCNE).
  • Experience: Previous experience in an acute care setting.
  • Licensure/Certification: Current and in good standing RN Licensure with the South Carolina Board of Nursing and American Heart Association Healthcare Provider course with Basic Life Support (BLS) certification.
  • Knowledge/Skills/Abilities: Works with patients, families, physicians, and other healthcare providers; proficient in using Electronic Health Record (EMR) for patient-level care and coordination; excellent verbal communication skills; ability to collaborate with others to improve quality and address needs.
