Position Summary:
This position involves providing professional nursing care to patients in an ambulatory clinic setting. The successful candidate will work collaboratively with interdisciplinary teams to develop and implement individualized plans of care that meet the unique needs of each patient. A valid Colorado RN license and current BLS certification are required upon hire.

Responsibilities:
1. Systematically collect and analyze data related to patient health needs and concerns.
2. Develop and implement individualized plans of care that address patient-specific needs and goals.
3. Provide education and support to patients and their families on various aspects of health and wellness.
4. Collaborate with other healthcare professionals to ensure seamless care coordination across the healthcare continuum.

Requirements:
Graduation from an accredited school of nursing, BSN preferred. Current Colorado Registered Nurse (RN) License and current BLS certification required upon hire. Ambulatory Care Nursing Certification preferred.
