About the Role:

We are seeking a skilled and empathetic individual to join our team as a Compassionate Care Coordinator. As a key member of our interdisciplinary team, you will be responsible for coordinating patient care, supporting patients and their families, and promoting a culture of compassion and understanding.

Key Responsibilities:

  • Conduct thorough assessments of patients' needs and preferences, identifying areas for improvement and opportunities for care enhancement
  • Develop and implement personalized care plans in collaboration with the healthcare team, incorporating patients' values and goals into care decisions
  • Provide education and support to patients and their families on end-of-life care, pain management, and related topics
  • Facilitate communication among healthcare providers, patients, and families to ensure seamless care coordination and resolve concerns promptly
  • Maintain accurate and timely documentation of patient interactions and care activities, ensuring compliance with regulatory requirements and UH documentation standards
  • Participate in interdisciplinary team meetings to discuss patient progress, plan adjustments, and share best practices
  • Serve as a resource and advocate for patients and families, addressing concerns and issues as needed and promoting patient-centered care
  • Collaborate with community resources to ensure access to necessary services and supports, such as home health care, adult day programs, and grief counseling

Requirements:

  • Master's degree in Social Work (MSW) from an accredited program
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) certification
  • Minimum 2 years of experience in palliative care, hospice, or a related field, with demonstrated expertise in care coordination and patient advocacy
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and build strong relationships with patients, families, and healthcare teams
  • Strong assessment, planning, and problem-solving skills, with ability to prioritize multiple tasks and manage time efficiently, working collaboratively with others to achieve shared goals
  • Proficiency in electronic medical records (EMRs) and other relevant software systems, with ability to learn new systems and technologies as needed
  • Valid driver's license and reliable transportation, with ability to travel to patient locations as needed

Salary and Benefits:

  • Competitive salary range: $62,500 - $85,000 annually, based on experience and qualifications
