Job Responsibilities and Qualifications
The Case Coordinator (CC) is responsible for delivering high-quality, patient-focused care by managing rehabilitative services to achieve optimal patient outcomes. The CC coordinates and directs care tailored to the individual needs of each rehabilitation patient, collaborating with the Rehabilitation Program Director and Facility Case Management Director.
Key Responsibilities:
- Provide orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and discharge criteria, including Medicare and insurance benefits.
- Facilitate patient and family education, encouraging active participation in discussions about care plans and goals.
- Ensure the execution of the patient’s treatment plan, focusing on their strengths, needs, and preferences.
- Document findings from discharge planning evaluations and psychosocial assessments, communicating any needs or preferences of the patient and family.
- Promote positive patient interactions that enhance the quality of service.
Qualifications:
- Registered Nurse with an Associate Degree or Certificate.
