Position Overview
The Family Empowerment Coach will engage and enroll families, conduct comprehensive assessments, and establish personalized objectives for youth and their families. This role involves regular evaluations of progress to ensure that plans align with participant needs, facilitating service connections, and maintaining follow-up support. The Family Empowerment Coach will work closely with the Program Manager and community collaborators to deliver client-focused care coordination. Responsibilities include leading parenting workshops, conducting individual family meetings, and performing home visits as necessary. Documentation of service activities through detailed case notes, conducting satisfaction assessments, and expanding the referral network are essential components of this role.
Key Responsibilities:
- Maintain a high standard of service delivery.
- Keep a Weekly Log of services provided.
- Document thorough and detailed Case Notes for each client.
- Track and report weekly service units to the Program Manager.
- Participate in and support monthly community engagement events.
- Conduct assessments in the field and ensure timely referrals to appropriate service providers.
- Maintain a daily journal of activities within the assigned service area.
- Ensure accurate and timely reporting of activities and progress towards goals.
- Effectively communicate both verbally and in writing.
- Respond to inquiries from the public and community stakeholders.
- Engage in physical activities related to community outreach.
- Complete assigned tasks within designated timeframes.
- Foster positive relationships with team members and community partners.
- Complete all required training promptly.
- Other duties as assigned.
Required Qualifications:
- Bachelor's degree in social work, human services, or a related field with a minimum of two years of relevant experience.
- Fluency in English (reading, writing, and speaking).
- Experience collaborating with community organizations.
- Understanding of community dynamics and social issues.
- Experience in juvenile assessment, counseling, or family intervention.
- Strong verbal and written communication skills.
- Ability to meet the physical demands of community outreach activities.
- Successful completion of a criminal background check.
- Self-motivated and proactive.
- Proficiency in Microsoft Office Suite.
- Detail-oriented with strong time management skills.
- Ability to work independently and take initiative.
- Must pass a Level II background check.
- Strong critical thinking skills.
Preferred Skills:
- Expertise in community engagement strategies.
- Experience in social service delivery.
- Bilingual abilities (Spanish or Creole are advantageous).
Core Competencies:
The Family Empowerment Coach role necessitates several key competencies. Proficient case management skills are crucial for developing and updating goals and coordinating services. Effective communication is essential for engaging with clients and partners. Accurate documentation practices are necessary for maintaining case records. Community engagement skills are vital for participating in outreach events and expanding service access. Data management capabilities are required for tracking progress and reporting outcomes. Teamwork and the ability to work autonomously are essential for fulfilling responsibilities effectively.
Physical Demands:
This position requires the ability to meet clients in person and engage in community activities. Responsibilities include attending outdoor events and facilitating community engagement efforts.
