Job Summary:

The Home Health LVN is responsible for providing skilled nursing care to patients in their homes, under the supervision of a registered nurse. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Administer skilled nursing care to patients in their homes, under the supervision of a registered nurse.
  • Participate in the coordination of care with the interdisciplinary team, patient, family, and referring agency.
  • Provide patient and caregiver education.
  • Assure the continuity of care through delivery of quality patient care.
  • Monitor assigned cases to ensure compliance with requirements of third-party payor.
Requirements:
  • Graduate of an accredited school of vocational nursing.
  • Active state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.
  • Current valid driver's license.
  • One year of experience as a licensed vocational/practical nurse in a clinical care setting required. Home health experience preferred.
Physical Requirements:
  • Prolonged standing and walking required, with ability to lift up to fifty pounds and move patients.
  • Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.
