About the Role

We are seeking a skilled Social Worker to join our team at MVP Health Care. As a Social Worker, you will play a critical role in providing psychosocial support to our members enrolled in the Medicaid Long Term Support Program.

Key Responsibilities
  • Assess and identify the needs of our members, providing education and counseling to support their well-being.
  • Coordinate and case manage services to ensure our members receive the necessary support to manage their chronic, acute, or disabling conditions.
  • Establish and maintain relationships with community agencies to provide contracted and alternative services when appropriate.
  • Refer members and/or families to community resources to assist in recovery from physical or long-term illness.
Requirements
  • Licensed Social Worker in NYS with demonstrated clinical experience and previous experience working with the Medicaid population.
  • Ability to solve problems while exhibiting clinical judgment and realistic understanding of the current environment.
  • Previous experience required in situations demanding strict adherence to confidentiality.
  • Ability to make independent decisions about appropriateness, resource utilization, and quality of care.
