Expert Hospice Nurse

The Westminster Canterbury is seeking an experienced Registered Nurse to join our team as a Hospice Nurse. As a key member of our Interdisciplinary Team, you will provide high-quality patient care and support to individuals in need of hospice services.
About the Role:
We are offering a competitive salary of $65,000 - $80,000 per year, depending on experience. In this role, you will be responsible for assessing and reassessing the nursing needs of patients, developing and implementing individualized care plans, and providing education and support to patients and their families.
Responsibilities:
* Assess and reassess patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
* Develop and implement comprehensive care plans that address patients' medical, social, and emotional needs.
* Provide direct patient care, including administering medications, performing treatments, and coordinating with other healthcare professionals as needed.
* Educate patients and their families on disease management, symptom control, and end-of-life care options.
* Collaborate with interdisciplinary teams to ensure seamless care transitions and optimize patient outcomes.
* Maintain accurate and up-to-date medical records, ensuring compliance with regulatory requirements and organizational policies.
Requirements:
* Current licensure as a Registered Nurse in the state of Virginia.
* Minimum 2 years of experience in hospice or palliative care settings.
* Strong assessment and critical thinking skills, with ability to prioritize tasks and manage multiple responsibilities simultaneously.
* Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and healthcare professionals.
* Ability to maintain confidentiality and handle sensitive information with discretion.
