About the Role:

  • Evaluate member needs and develop personalized care plans to ensure quality, cost-effective healthcare outcomes.
  • Collaborate with healthcare providers and partners to identify resources and services that meet member needs.
  • Provide education and support to members and their families/caregivers on procedures, service options, and healthcare benefits.
  • Monitor care plans and provide recommendations for improvement.
  • Perform home visits and site assessments to evaluate member needs and collaborate with healthcare providers.

Requirements:

  • Bachelor's degree in a related field or equivalent experience.
  • 2-4 years of experience in long-term care management or a related field.
  • LVN/LPN or Bachelors in Social Work preferred.
  • Candidate must reside in Nueces County due to face-to-face visits with members.

Compensation and Benefits:

We offer a competitive salary of $65,000 - $85,000 per year, based on experience, plus a comprehensive benefits package including health insurance, 401K, stock purchase plans, tuition reimbursement, paid time off, and a flexible work schedule. Total compensation may also include additional forms of incentives.
