About the Role

We are seeking a highly skilled and compassionate Licensed Clinical Social Worker LCSW to join our team in El Paso, TX. As a Licensed Clinical Social Worker LCSW, you will play a vital role in providing social and emotional support to our clients, helping them navigate their recovery journey.



Key Responsibilities

  • Provide direction, supervision, and evaluation of medical social services to patients in the home, ensuring compliance with state practice acts and company policies.
  • Instruct, treat, observe, and evaluate patients with significant social and emotional problems affecting their health status.
  • Assist physicians and other healthcare team members in understanding the social and emotional factors related to patients' health problems.
  • Participate in the development and periodic re-evaluation of the physician's Plan of Treatment for patients needing social work services.
  • Utilize special skills and judgment to resolve social and emotional responses connected with illnesses.
  • Observe, record, and report changes in patients' condition and response to treatment to the Clinical Manager and physician.
  • Participate in discharge planning and provide ongoing assessment of patient and family needs and responses to teaching.
  • Supervise, instruct, and evaluate the performance of the Social Work Assistant (BSW) to ensure compliance with company and government standards.
  • Maintain and submit documentation as required by the company and/or facility, including case conferences, patient/physician/community contacts, visit reports, progress notes, and confers with other healthcare disciplines in providing optimum patient care.
  • Attend, participate in, and/or conduct internal staff development programs, obtain continuing education as required by company policies and regulations, and maintain the highest standards of professional conduct in relation to confidential information.


Qualifications

  • Bachelor's Degree required; Master's Degree preferred from a school of social work accredited by the Council on Social Work Education with six months' social work experience in a healthcare setting.
  • Current licensure/certification in the state(s) of practice if required.
  • CPR certification required.
  • Successful completion of any/all company required pre-employment competency testing and health clearance prior to assignment and periodically as required by company policy and state regulations.
  • Good organization and communication skills and ability to function with minimal supervision also required.
