Job Summary

We are seeking a highly skilled and compassionate Home Health Social Worker to join our team at CommonSail Investment Group. As a Home Health Social Worker, you will play a critical role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Assess the psychosocial status of clients to determine factors that may interfere with their ability to achieve goals.
  • Develop and implement written plans to provide information and direction to other healthcare team members.
  • Document all findings, plans, interventions, and client progress in the clinical record.
  • Communicate plans and changes to the physician and/or nursing supervisor and other healthcare staff through the care plan, written progress notes, and participation in care conferences.
  • Initiate referrals as appropriate and prepare social histories to augment existing service or as a guide in determining or changing level of service.
Requirements
  • Master's Degree from a school of Social Work accredited by the Council on Social Work Education.
  • Minimum of one (1) year's experience in a healthcare setting or equivalent experience.
  • CPR certification, as applicable.
  • Demonstrates knowledge of resources available in the community.
  • Licensed driver with automobile insured in accordance with state/Agency requirements.
