We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Plymouth. As a Private Duty LPN, you will provide high-quality nursing care to patients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities- Provide direct nursing care to patients in their homes, as prescribed by physicians
- Record clinical and progress notes, and coordinate other patient care activities to provide the highest quality of patient care
- Work under the direct supervision of a Registered Nurse and/or designated supervisor
- Ensure confidentiality of patient health information and maintain accurate records
- Current LPN licensure in the state of practice
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
- Current CPR certification
- Previous home care experience preferred
