Job DescriptionPhoenix Senior Living is seeking a skilled Geriatric Healthcare Specialist to join our care team. In this role, you will be responsible for providing high-quality care and support to our residents, promoting their health and wellbeing.
Key Responsibilities:
- Monitor resident health and wellbeing, identifying areas for improvement and implementing effective interventions.
- Collaborate with the care team to develop and implement personalized care plans that meet resident needs.
- Provide education and support to family members and caregivers on resident care and management.
- Participate in ongoing quality assurance initiatives to improve care delivery and outcomes.
Requirements:
- Certified Medication Aide (CMA) certification required.
- A minimum of one year of experience working with elderly or disabled individuals preferred.
- Demonstrated leadership capabilities and ability to work collaboratively with others.
