Job Summary

We are seeking a highly skilled Registered Nurse to join our team at The Oaks at Jamestown. As a Registered Nurse, you will be responsible for providing high-quality patient care, developing personalized care plans, and collaborating with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Provide Direct Patient Care: Administer medications, perform wound care, and provide emotional support to residents and their families.
  • Develop and Implement Care Plans: Assess resident needs, develop personalized care plans, and collaborate with the interdisciplinary team to ensure effective implementation.
  • Collaborate with the Interdisciplinary Team: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive care and effective communication.
  • Monitor and Document Resident Progress: Track resident progress, document care provided, and report any changes or concerns to the interdisciplinary team.
  • Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives, participate in training and education programs, and engage in continuous learning to stay up-to-date on best practices.
Requirements
  • Current RN License: Possess a current and valid RN license in the state of Michigan.
  • Experience in Long-Term Care: Have experience working in long-term care settings, preferably in a similar role.
  • Strong Communication and Interpersonal Skills: Possess excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and the interdisciplinary team.
  • Ability to Work in a Fast-Paced Environment: Demonstrate the ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
