Job Summary:

Erickson Senior Living is seeking a skilled Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join our team as a Clinical Care Specialist. In this role, you will provide high-quality clinical care to our residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities:

  • Deliver exceptional clinical care: Assess, observe, and evaluate resident clinical conditions, documenting findings in the electronic medical record and notifying providers and RNs as needed.
  • Develop and implement care plans: Update or modify care plans to reflect changes in resident care and service needs, ensuring seamless transitions and continuity of care.
  • Lead and supervise care associates: Provide guidance, support, and oversight to care associates, promoting a culture of excellence and resident-centered care.
  • Administer medications: Timely and accurately administer medications according to orders, ensuring resident safety and well-being.
  • Participate in quality initiatives: Collaborate with the interdisciplinary team to review clinical indicator reviews, falls, wounds, nutrition, antipsychotic use, and other key metrics, driving continuous quality improvement.
  • Communicate effectively: Respond promptly and professionally to resident, family, and responsible party inquiries, ensuring clear and concise communication.

Requirements:

  • Graduate of an approved LPN/Vocational Nursing program.
  • Minimum 1 year of experience in assisted living, home health, or long-term care.
  • Passion for caring for seniors and advocating for resident needs.
  • Experience with electronic medical records preferred.
