Job Title: Licensed Practical Nurse

Brockton Area Multi-Services, Inc. (BAMSI) is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Easton, Massachusetts.

Job Summary:

We are looking for a compassionate and detail-oriented LPN to provide high-quality patient care to individuals with complex medical needs, developmental disabilities, acquired brain injury, and mental health issues.

Responsibilities:
  1. Administer medications and treatments in accordance with Department of Public Health's Medication Administration Program (MAP) policies and other funder regulations.
  2. Complete daily written notes, data tracking, and skilled assessments to ensure accurate and timely documentation.
  3. Follow medical protocols, treatment plans, and medication treatment plans to provide comprehensive care.
  4. Communicate health changes in a timely manner to ensure seamless care coordination.
  5. Utilize Positive Behavior Supports (PBS) practices to create a supportive work environment that promotes growth, dignity, and independence.
  6. Provide physical support, assistance with lifting and transferring individuals served who have physical challenges as needed.
  7. Work collaboratively with direct care and supervisory teams to ensure the health status of individuals.
  8. Provide oversight of meals, ensuring food and meals are prepared properly, and train staff on meal preparations and specialized feeding protocols as needed.
  9. Provide ADL care, transportation, and community outings as needed.
  10. Assist with the maintenance of medication books, house log books, etc. to ensure program compliance with all regulations.
  11. Ensure medications, medical, and safety supplies are ordered and available as needed.
  12. Follow admission and discharge requirements for hospitalized individuals.
  13. Provide training to both staff and individuals served in relevant areas (e.g., health, hygiene, safety, diet, etc.).
  14. Provide holiday coverage on a rotating basis.
  15. Attend staff meetings.
  16. Provide coverage at day habilitation programs or other residential programs as needed.
  17. Ensure safety standards and infection control protocols are followed.
  18. Perform other related duties as needed or as assigned by supervisor or designee.
Requirements:
  • Possession of Massachusetts license as a nurse.
  • Must possess a valid driver's license in state of residence and 3 years of driving experience.
  • Basic computer literacy, including the ability to record data, use electronic time and attendance software, and receive and send email, is required.
Preferred Qualifications:
  • 2 years experience working with individuals with acquired brain injury or similar disabilities.
