Responsibilities:
  • Evaluate patients and develop individualized treatment plans to achieve optimal outcomes.
  • Conduct ongoing assessments and modify treatment programs as needed to ensure timely progression.
  • Collaborate with other healthcare professionals to coordinate patient care and ensure a seamless continuum of services.
  • Provide patient education and training to empower individuals to manage their conditions and achieve independence.
  • Develop and implement comprehensive discharge plans to ensure a safe and effective transition to community settings.
  • Identify and manage various levels of care to ensure delivery of high-quality patient care.
  • Supervise and delegate tasks to Physical Therapist Assistants and support staff to maximize patient outcomes.
Qualifications:
  • Current state licensure or eligibility for licensure as a Physical Therapist.
  • Certified BLS or completion within the first 90 days of employment.
