The Program

Vista Hill SmartCare is a unique program that offers integrated care and support services to patients in rural medical clinics. Our team of professionals works collaboratively with local healthcare providers to identify community health needs and develop innovative solutions to address them.

The Position

We are seeking a highly skilled Behavioral Health Consultant to join our team. As a key member of our SmartCare program, you will work under the supervision of a Licensed Behavioral Health Supervisor/Program Manager to provide behavioral health care to patients in the health clinic setting. Your primary responsibilities will include:

  • Providing brief, strategic behavioral health interventions in the clinic, community, and patients' homes
  • Coordinating with the Primary Health Care team to ensure a seamless and integrated experience of care
  • Developing and implementing curricula for activities and classes such as stress management, healthy relationships, and substance abuse prevention
  • Responding quickly and appropriately to crisis situations
  • Maintaining accurate and timely documentation of patient services and outcomes
Requirements

To be considered for this role, you must have:

  • A Master's degree in a relevant field
  • Current licensure or registration as an LCSW, LMFT, or LPCC with the California Board of Behavioral Science
  • Experience working with health and wellness issues for adults and children
  • Bilingual skills in English and Spanish (preferred)
  • Knowledge of East County rural communities (preferred)
