About the Role:

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Home Health Speech Language Pathologist, you will provide high-quality speech and language therapy services to patients in their homes, helping them to communicate effectively and improve their overall quality of life.

Key Responsibilities:

• Evaluate and develop individualized treatment plans for patients with speech and language disorders
• Provide one-on-one therapy sessions to patients in their homes
• Collaborate with other healthcare professionals to ensure comprehensive care
• Maintain accurate and detailed records of patient progress and treatment plans
• Participate in care coordination activities and discharge planning

Requirements:

• Master's degree in Speech-Language Pathology or related field
• Certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) or eligible
• Minimum of six months experience as a Speech-Language Pathologist
• Home Health experience preferred
