Medical Facilities of America is seeking a highly skilled and dedicated MDS Coordinator to join our team at Albermarle Health & Rehabilitation Center. As a key member of our interdisciplinary team, the MDS Coordinator will play a critical role in ensuring the delivery of high-quality patient care.
**Key Responsibilities:**
- Conduct comprehensive patient assessments and develop individualized care plans in accordance with federal and state regulations, as well as company policies and procedures.
- Collaborate with direct care staff, physicians, and family members to ensure seamless communication and coordination of patient care.
- Utilize strong clinical assessment skills to identify patient needs and develop effective care plans.
- Develop and maintain accurate and comprehensive patient records, ensuring compliance with regulatory requirements.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
**Requirements:**
- Current RN license and experience in a skilled nursing and rehabilitation environment.
- Strong clinical skills and knowledge of the MDS 3.0 process.
- Excellent communication and interpersonal skills, with the ability to work effectively with interdisciplinary teams.
