Job Title: Clinical Case Manager Full Time Permanent

We are seeking a skilled Clinical Case Manager to join our team at RN-Staff. As a Clinical Case Manager, you will play a vital role in planning, organizing, and coordinating swing bed admissions and reviewing emergency department admissions and discharges.

Responsibilities:
  • Provide administrative and clerical services to support the clinical team.
  • Contribute to process improvement initiatives to enhance patient care and outcomes.
  • Coordinate with the referral specialist to ensure seamless transitions of care.
  • Manage, obtain, and share medical records to facilitate informed decision-making.
  • Work collaboratively with the interdisciplinary team to develop and implement patient care plans.

Requirements:
  • Three to five years of experience in clinical nursing.
  • One to two years' experience in case management or a related field preferred.
  • Current licensure to practice in the state of California.
  • Bachelor's degree in nursing highly preferred.
  • BLS required, ACLS preferred.
