Key Responsibilities- Clinical Assessments and Patient Care Plans: Perform thorough clinical assessments, update patient care plans, and complete relevant sections of the Minimum Data Set (MDS).
- Medicare PDPM System Knowledge: Maintain a working knowledge of the Medicare PDPM system to ensure accurate capture of patient-related needs.
- Electronic Medical Record and Meal Planning Software: Document patient information in our electronic medical record system (PointClickCare) and utilize our meal planning software to ensure proper patient meal needs are met.
- Collaboration with Food Service Team: Work closely with our facility chef and dietary department to promote optimal food service operations, attend meal services, and participate in solving meal service issues.
- Dietary Cost Control: Collaborate with our facility to maintain dietary cost control by balancing nutritional care with the use of supplements and other nutritional interventions.
- Communication and Collaboration: Communicate effectively with patients, families, physicians, and the Interdisciplinary Team (IDT) to promote self-directed nutritional care and provide real foods first.
- Problem-Solving and Leadership: Demonstrate a professional and leadership role by representing yourself as a leader in the field of nutrition, effectively solving problems related to nutrition and dietary, and working closely with the food service manager to assist with food service issues.
- Licensure: Must be licensed in the state in which you practice.
- Attention to Detail: Ensure processes are completed timely and accurately.
