Job Title: Licensed Practical Nurse

At Trustwell Living of Park, we are seeking a skilled and compassionate Licensed Practical Nurse to join our team. As a key member of our healthcare staff, you will play a vital role in providing high-quality care to our residents.

Job Summary:

We are looking for a dedicated and experienced Licensed Practical Nurse to provide direct resident care and supervision of resident needs. The successful candidate will be responsible for monitoring residents for changes in condition and signs/symptoms of illness, communicating and documenting resident changes or concerns to the Health Services Director, and assisting in the supervision of resident assistants.

Responsibilities:
  • Comply with all federal and state laws, including state Resident Rights' law.
  • Comply with all Community policies and procedures.
  • Monitor resident health status and ensure resident privacy and dignity.
  • Assess, document, and report changes in resident condition.
  • Provide emergency medical intervention as needed and administer medications/treatments per community policies/procedures.
  • Ensure implementation of resident's individualized care plan and provide recommendations to the Director of Wellness for changes to the care plan.
  • Communicate resident care plan changes to resident assistants and document resident medical data per community policy.
  • Document new physician orders and communicate changes to nursing staff, monitor ADL documentation per community policy, and document, investigate, and timely report all resident incidents.
  • Immediately intervene, report, and investigate suspected or alleged verbal, physical, sexual abuse, or theft from resident, maintain safe storage of medication, and review monthly weight/vital sign documentation.
  • Implement and monitor behavioral management interventions, assist residents with daily ADLs, maintain confidentiality of residents, families, and community, and maintain a safe and clean environment for residents and staff.
  • Practice universal precautions, comply and oversee infection control procedures per community policy, and report safety concerns and maintenance issues to supervisor.
Requirements:
  • Willingness to adhere to assisted living principles in provided services.
  • Ability to utilize and apply basic management principles.
  • Ability to recognize and communicate problems effectively.
  • Ability to relate to the public, residents, families, staff, and other professionals appropriately.
  • Ability to communicate well, verbally and in writing.
  • Ability to maintain accurate records.
  • Ability to adhere to and communicate policy and procedures.
  • Ability/willingness to improve skills of self and staff; to be active professionally.
  • Ability to lift to 50 pounds occasionally, 30 pounds regularly.
  • Maintains appropriate and required license.
Training and Experience:

A Licensed Nurse with a minimum of at least one year in-home health or geriatric nursing in a similar position. Rehabilitative and restorative experience helpful. CPR/First Aid certification required.
